The Support at Home End-of-Life Pathway
The Australian Government Support at Home End-of-Life Pathway provides additional short-term aged care funding for eligible older Australians approaching the end of life who wish to remain at home.
This pathway is designed to work alongside, rather than replace, specialist palliative care and state or territory health services.
