Transformative Nursing & Support Services is proud to provide high-quality nursing and in-home support to Australian veterans and their families through the Department of Veterans Affairs (DVA) Home Care Program. Our experienced team understands the unique needs of those who have served our country and is committed to delivering compassionate, dignified care.
What We Provide
DVA Community Nursing
Registered nurses providing clinical care in your home, funded through the DVA Home Care Program.
Wound Management
Specialised wound assessment and evidence-based dressing management by experienced nurses.
Medication Management
Assistance with medication administration, dosette box packing, and medication reviews.
Post-Surgical Care
Skilled nursing support following surgery to promote safe recovery in the comfort of home.
Personal Care Assistance
Support with daily living activities including bathing, grooming, and mobility assistance.
Transport to Medical Appointments
Safe, reliable transport for veterans attending specialist and medical appointments.
Who Is Eligible?
DVA-funded services are available to eligible Australian veterans, war widows/widowers, and dependants who hold a DVA Gold Card or White Card (for accepted conditions).
Services are provided under the DVA Home Care Program and may include nursing, personal care, transport, and other community support services — at no cost to eligible card holders.
